Monitoring, patching, and documentation live in one place both sides can see. Your team is not asking us what changed, and we are not asking your team where something is.
If you already have tools your team is happy in, we work there. If you do not, ours become the shared record. Either way you keep access to what is in it, including if we stop working together, because a record of your own environment that you cannot take with you is not much of a record.
The side effect is the one clients notice most: your business stops depending on one person's memory. That is worth having whoever is doing the work, and it is the thing that makes a resignation survivable.

No, we start from it. Most of what a working IT person has written down is accurate and worth keeping. The gap is usually that it stops at the edge of what that one person deals with.
Can our team see everything you see?
Yes. A co-managed arrangement where one side can see the monitoring and the other cannot is the ambiguous boundary that causes the trouble in the first place.
